Required Documentation

  • Insurance Card (Original and Copy)

    Please bring your current insurance card and a photocopy for our records.

  • Valid Photo ID

    Government-issued photo identification such as Aadhar card, driving license, or passport.

  • Pre-authorization Form (if applicable)

    Some insurance plans require pre-authorization for specific procedures or services.

  • Referral Documentation

    If your insurance requires a referral to see a specialist, please bring the referral documentation.

Cashless Facility

We offer cashless treatment facilities with our network insurance providers. This service allows you to receive treatment without making upfront payments (subject to policy terms).

Cashless Process

  1. Submit your insurance details at admission
  2. Our insurance desk will verify your eligibility
  3. We'll obtain pre-authorization from your insurer
  4. Treatment proceeds without upfront payment
  5. At discharge, you pay only for non-covered items
Note: If your insurance company does not approve the pre-authorization request or if certain procedures are not covered under your policy, you may be required to pay those expenses.

First-Time Visit Checklist

Laboratory Test Preparation

  • Follow fasting requirements if specified (usually 8-12 hours for blood tests)
  • Stay hydrated with water unless instructed otherwise
  • Bring your lab requisition form and identification
  • Inform the lab of any medications that may affect results

Imaging Exam Preparation

  • Follow specific preparation instructions for your exam type (CT, MRI, Ultrasound, etc.)
  • Remove all metal objects before MRI (jewelry, watches, etc.)
  • Wear comfortable clothing without metal fasteners
