Visit Checklists
Preparation guidelines to help you make the most of your visit and ensure everything goes smoothly. Use these checklists to prepare for your specific appointment type.
First-Time Visit Checklist
- 1Complete and bring all new patient forms (or arrive 20 minutes early)
- 2Bring your valid photo ID and insurance card
- 3Bring a list of all current medications (including supplements)
- 4Bring relevant previous medical records and test results
- 5Prepare a list of questions or concerns to discuss
- 6If applicable, bring any referral documentation from other doctors
- 7Arrive at least 15 minutes before your scheduled appointment
Laboratory Test Preparation
- Follow fasting requirements if specified (usually 8-12 hours for blood tests)
- Stay hydrated with water unless instructed otherwise
- Bring your lab requisition form and identification
- Inform the lab of any medications that may affect results
Imaging Exam Preparation
- Follow specific preparation instructions for your exam type (CT, MRI, Ultrasound, etc.)
- Remove all metal objects before MRI (jewelry, watches, etc.)
- Wear comfortable clothing without metal fasteners
- Inform technician of any implants, devices, or pregnancy
Surgery Preparation
- Complete all pre-surgery testing as scheduled
- Follow all fasting instructions precisely
- Arrange for someone to drive you home after surgery
- Stop certain medications as advised by your doctor
- Bring all required paperwork and identification
